A note on confidentiality

You do not need to give your name or any contact information to submit this form — it can be completed entirely anonymously. What you share is stored securely and access is limited to a small number of designated staff who handle rapid response cases. We do not record your IP address or other identifying technical information.

After you submit, a member of our Rapid Response team will review what you've shared. If you've left contact information and the situation calls for it, we may reach out — but only if you've told us how, and only with care for your safety.

If you are in immediate physical danger, please also contact local emergency services or the police directly — this form is reviewed as soon as possible, but is not a substitute for emergency response.
